Urgent Care Positions at Practice VelocityOnline Application for Internet Email UsersDirections: If you do not have Adobe Reader on your computer, please download it by clicking on the Adobe Reader button below. After Adobe Reader is installed: 1. Click on the Practice Velocity application link below. 2. Fill out the entire application form. The required fields outlined in red must be fully completed even if supplemented with a resume. Incomplete applications may not be considered! 3. If you do not have time to finish it, you can save it to your computer at any time by selecting File --> Save As --> PDF. Please rename it with your initials at the end (i.e. PVEmpAppForm2TLM.pdf). 4. When form is completely filled out, save the file as in number 3. 5. Go to your internet email website (Hotmail, Gmail, Yahoo etc.). 6. Create a new email to jobs@practicevelocity.com 7. Attach the completed from as a file to the email. 8. Hit Send.
